How to apply for a new line:
1. Fill out the "New Phone Line Application Form" and press the 'Send Form' button. All data will be stored in the web site database.
2. TOT officer contacts the applicant to confirm the application.
3. After receiving contact, please go to any TOT office in your area with the following documents to apply for a new telephone line.
For Personal Phone Line
A copy of Personal ID card/Passport or house registration which is signed for authentication
For Bussiness/Organizational Use
A copy of Business Registration Certificate, or Certificate of Incorporation for application under the name of firm/company, which does not exceed a 90-day period
A copy of personal ID card/Passport of authorized person acting under the name of firm/company
All documents must be officially stamped and signed for authentication.
4. Submit all required documents at TOT office according to appointment.
Note :
In case the person applying for new line is not available to process the request, additional documents are required as follows:
- A proxy for application under the name of that person with tax stamp
- A personal ID/Passport of the proxy
Terms and Conditions
The subscriber agrees that this request is a telephone service agreement effective immediately after the provider agrees to provide phone service to the subscriber. Hereby, the subscriber accepts the following terms and conditions.
Telephone Service Agreement 1. The TOT Public Company Limited hereinafter called "Service Provider". 2. The applicant whose subscription has been approved by the Service Provider hereinafter called "Subscriber". 3. The Subscriber shall follow the regulations related to services set forth by the TOT Public Company Limited. Any changes in regulations, which shall not affect the benefits of both the service provider and subscriber according to this agreement, will be announced to public at least 30 days in advance. 4. The subscriber is in charge of providing and maintenance of the telephone and equipment where the installation is taken. 5. The subscriber shall settle payment of service and/or fee within 7 days after the received date of invoice. 6. In the event that the outstanding balance of a telephone number is not settled, the service provider holds the right to discontinue the service for that telephone number, provided that the service provider will make notification either in written form or other means of communications at least 15 days in advance. 7. The service provider shall return the deposit to the subscriber within 47 days after the agreement has been terminated, provided that the subscriber does not have any outstanding balance. In the event that the outstanding balance is left unpaid, the subscriber shall allow the service provider to debit the outstanding balance from the deposit. If the outstanding balance is a larger sum than the deposit, the service provider holds the right to debit the deposit of other telephone numbers in the same agreement. However, the subscriber is obliged to pay the required deposit amount in order to continue the service of the remaining telephone numbers within 15 days after the receiving notification from the service provider. 8. This telephone service agreement grants the right to specified subscriber. The transfer of subscription to other party will not be effective until the written consent by the service provider was made. 9. The service provider reserves the right to change the telephone number, telephone system or the way of conveying services provided. In doing this, the service provider shall inform the subscriber at least 30 days in advance. 10. When problem with the line arises, the subscriber shall inform the service provider so that the service provider could manage to have the line and connection fixed within an appropriate amount of time. The recovery service is free of charge, except the problem/damage caused by the subscriber or persons under the subscriber's custody. The subscriber shall not claim for any compensation from the service provider in case of any damages caused by the telephone usage problem. 11. Any written materials, letters, or word of mouth delivered from the service provider to the subscriber's billing address are deemed to be rightfully acknowledged by the subscriber. 12. If the subscriber breaches any one of the conditions set forth in this agreement, violates laws or regulations related to telephone usage, either at present or in the future, or violates any other laws and commits anything against national security, the service provider has the right to discontinue the service and/or terminate the agreement. In this case, the service provider shall notify the subscriber of the reason of termination at least 15 days in advance. In the event that the subscriber wants to discontinue the service, the subscriber shall send the written notification to the service provider at least 30 days in advance.
The subscriber agrees to be bound by and subject to these terms and conditions. And the subscriber is entirely responsible for all obligations incurred from the use of service.
